Your visit

If you already have an appointment, we are excited and look forward to seeing you and having you in our practice.  We want to do everything possible in preparing you for your first visit with us.

This is what to expect:

Your eyes will be dilated and special testing may need to be done, so plan to be with us for at least two hours.  We will provide you with disposable sunglasses for your comfort, but ask that you make transportation arrangements if you find it difficult to drive after your eyes have been dilated.  We welcome your spouse, family members, or a friend to join you during your visit with us.

Please bring your completed “new patient” forms on your first visit.  These forms can be downloaded from the list of forms at the bottom of this page or we can mail them to you if you prefer. 

It is very important that you bring with you a list of all medications that you are currently taking, including any eye drops that you are using.

At your initial visit, all insurance cards and a picture ID are required.  Without these, your appointment may need to be rescheduled.

You can download new patients forms here to complete them prior to your visit.

If your insurance requires an authorization from your primary care physician, you are responsible to provide us with that form upon check-in.  Deductibles and co-payments are your responsibility and will be collected at the time of your visit.  If you are unsure of your insurance coverage, please contact our office prior to your appointment so that we can check for you.

Monday:  Surgery Day

Tuesday thru Friday: – 8:30AM to 5:00PM
Saturday: – 8:30AM to 12:00PM

We ask that you be on time for your appointment, allowing Dr. Nemiroff adequate time for your examination.  If you are unable to keep your appointment, a 24-hour notice is required.
